Tuition Reimbursement Form
This form should be completed and submitted to your immediate supervisor prior to attending educational classes at an accredited college or school. Please refer to the employee handbook for the full tuition reimbursement policy.

Employee Withholding Certificate (Form W-4)

Direct Deposit Authorization
Complete this form if you would like to have part or all of your paycheck directly deposited in to a bank account (savings or checking). Submit the completed form to the payroll department. Please note that it may take up to two weeks for the direct deposit to take affect.

Vision Claim Form
Complete this form when you have received vision services and are requesting a reimbursement. You must currently be enrolled in the Lifestyles Vision Plan in order to receive reimbursement. See Human Resources for information about open enrollment periods.
